It's About YouOnce you have made the decision to set an appointment with Serenity Marriage and Family Counseling, LLC you may contact us through our online link below or call our offices at 918-970-0095 and speak to Whitney. We will use our knowledge and skills to help you with your needs. Counseling is typically provided on a weekly basis and is classified as a self pay fee for service. Payment is due at the end of service. I can provide services online or via phone call if you would prefer. Cancellation policy: Serenity Marriage and Family Counseling, LLC has a 24 hour cancellation policy. If an appointment is missed, cancelled or changed with less than 24 hours notice, the full session fee will be charged. Please confirm the day before for the appointment to be kept. Emergency situations will be resolved on a case by case basis. Clients are seen by appointment only. If your appointment is cancelled or rescheduled two times in a row (or twice) a referral will be made to another company. Appointment must be confirmed the day before. I ask that you show up to your scheduled appointment within the 10 minute grace period, or the appointment will be rescheduled and you will be charged the full amount of the service. If you "No Show" on the first session you will not be able to reschedule. Building a rapport is important in the therapeutic process and managing time is of great importance to me. Payment is due at the time services are rendered. If invoices are sent they are to be paid within 24 hours. If payment becomes an unresolved issue a referral will be made to another company. |
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059. |
